We are proud to announce that Seaside Home Health Care is now a contracted VA provider, allowing eligible Veterans to receive high-quality home health services in the comfort and safety of their own homes.

Eligible Veterans may qualify for a variety of in-home healthcare services through VA-authorized care, including:

  • Medication Management
  • Wound Care
  • Chronic Disease Management
  • Care Coordination & Case Management
  • Post-Hospital Recovery Support

Veterans may be referred for home health services by:

  • VA Physicians
  • VA Case Managers
  • VA Social Workers
  • Hospital Discharge Planners
  • VA Clinics and Specialists

Once approved through the VA Community Care program, eligible Veterans can receive services from contracted providers such as Seaside Home Health Care.
