What is Concierge Care Management?
Concierge Care Management is a way for seniors to have 24/7 access to a Professional Care Manager who will act as local “eyes and ears” to help provide insight and advice for managing their health and care needs.

- Personal & professional management of daily tasks
- Planning, scheduling and attending doctor visits
- Patient advocacy
- Crisis aversion/Intervention
- On-going care need assessments
- Consistent review of care goals and progress
- Weekly in-person visits from a dedicated care manage
